CHW Job Descriptions

The CHW works directly with community members in need and works with community partners to build systems and relationships. The CHW will address community issues related to community wellness, food security, substance misuse, and behavioral health. Through relationship building they will assess the needs of individuals and/or within the community, make referrals, and create referral systems. The CHW will provide linkages to services for community members; along with providing information and education to the community, gathering information to understand systems, community needs and resources.
