CHW Job Descriptions

Our organization is seeking a Community Health Worker to work directly with community members in need.  They will assess current living situation, make referrals, and provide linkages to services for community members; along with providing information and education to the community, gathering information for understanding of systems and operations needs and resources. Community Health Worker will take on a variety of tasks including providing support and education regarding nutrition/food security, diabetes, crib safety, car seat safety, housing, family planning information, opioid awareness training and support and referrals in those areas also.
